CMA Qualification Certification Application Materials

1. Application materials required for CMA certification by independent legal entity inspection and testing institutions (foreign-funded inspection and testing institutions shall refer to this item):

"Application Form for Qualification Certification of Inspection and Testing Institutions" (1 copy, with an electronic text in the format published on the Provincial Quality Supervision Bureau's website);

Business license of the legal entity, institutional legal person certificate, or other proof of legal entity (the business scope should include inspection and testing services);

In regions where the commercial registration system reform has been implemented, the business scope information shall also be submitted (1 copy, original required for verification);

Organization code certificate (1 copy, original required for verification; in regions where the business license and organization code certificate are combined into one document, submit the business license of the legal entity);

Proof of fixed premises ownership/usage rights;

Proof of labor relations for professional technicians and management personnel, i.e., one copy of social insurance proof confirmed with a seal by the social security department (original required for verification, and proof of three months or more of social insurance is required);

Proof of independent allocation of inspection equipment (1 copy, original required for verification);

Typical inspection reports or certificates (one for each category) (Note: applicable for first-time and expanded scope applications; additional copies may be required as per the review team leader's request);

Records of internal audits and management reviews of the management system (applicable for first-time and renewal reviews);

Qualification certificates for personnel engaged in inspection and testing in special fields (if applicable).

All the above materials must bear the official seal of the inspection and testing institution (legal entity).
